What We Do

At Soft Soles Advanced and Diabetic Foot Care, we are both registered nurses (RNs) and registered practical nurses (RPN). We work in hospitals, clinics, communities, and facilities. Universally, what we see is situations where independence and health deteriorates or is lost due to lower limb disease. We have partnered with facilities and clinics to decrease this risk and hope to help you as well as your family maintain lower limb health.

Our Team

Michelle De Grandmont, RPN, Owner, Nurse Educator

Michelle De Grandmont is a bilingual registered nurse. She has taken advanced diabetic foot care nurse training. Her work experience includes over 15 years of experience in gerontology, acute care medicine, diabetes, and holistic healing modalities. 


Michelle was an educator at Algonquin College, where she developed an innovative and highly successful advanced diabetic foot care program. With the closing of the program in 2018, Foot Canada Training partnered with Michelle to lead their Ottawa Foot Care Nurse Clinical courses and advanced skills workshops. Michelle is a frequent motivational speaker on best practice adherence in nursing foot care and works hard to keep the best practices in the community and advancing foot care nurse as a specialty.

Michelle is a volunteer Peer Advisor for The Canadian Association of Foot Care Nurses, where she has worked on the development of the National Foot Care Nurse Competencies Document and The Education Initiative.


Soft Soles takes a three-pronged approach to foot care by meeting the lower limb health care needs of communities, connecting individuals to local organizations and resources, and offering new foot care nurses’ employment opportunities, professional support and mentoring.

Through the delivery of foot care clinics in the Ottawa Area, Soft Soles carries out its mandate to provide knowledge and tools to maintain and foster independence.

Michelle and her partner live on a hobby farm with their five boys in Dunrobin, Ontario.

Amber Lawford, RN, Advanced Diabetic Foot Care Nurse

Amber has been a Registered Nurse in good standing with the College of Nurses of Ontario since 1998. Specializing in acute care medicine, and community health, Amber brings a passion for teaching and administering care to clients in their home.


Amber employs a holistic approach to each person she meets, enjoying teaching, sharing of stories, and forming quality relationships. Amber carries advanced diabetic foot care certification and provides excellent gold standard service to all of her clients.


Outside of nursing, Amber is an avid reader, quilter, scrapbooker, and a novice guitar player. She enjoy physical activities such as running, hiking with her dog, and staying active with her family. Communication skills and humor are the highlights of her personality.

Clara Provenzano, RPN, Advanced Diabetic Foot Care Nurse

Clara joins Soft Soles after having spent 8 years in health care and truly found her love for nursing while working in direct patient care for family physicians. Clara is passionate when it comes to preventative care and education.


Specializing in the care of the elderly and those with diabetes, she has discovered that many people need foot care, education in proper foot hygiene, and how that effects their independence. Clara graduated with honors from The Algonquin College one-year advanced diabetic foot care certificate program, Clara is an avid learner and is currently pursuing her Certification as a Diabetes Educator, Wound Care specialists and her RN BScN!

Jane Coleman, RPN, Advanced Diabetic Foot Care Nurse

Jane joins Soft Soles with an appetite to learn. Prior to nursing she achieved an honors degree in Zoology from the university of Guelph! 28 years ago, Jane went to St. Lawrence College in Brockville to get her RPN.


She has worked in long Term care, and spent time as a Central Supplies Reprocessing Clerk (CSR) She understands the critical importance of infection control and process. Jane loves to take her time with her patients and after 12 years as a community visit nurse she understands the importance of compassion, trust, and professionalism.

We are lucky to have her join our team!

Jordyn Mask, RPN, Advanced Diabetic Foot Care Nurse

Jordyn joins Soft Soles as an experienced nurse. Her work history has built her diverse knowledge in areas such as Med/Surg, Chronic Care, Dialysis, Emergency Medicine, Vascular, General Surgery, Plastic Surgery, Dermatology, as well as Infectious Diseases.


Having worked with many diverse at-risk patients Jordyn has come to rely on prevention to protect her patients from the illnesses she has seen. "I love my profession and enjoy working with and caring for people."


Soft Soles is fortunate to have such a passionate and knowledgeable care giver on our team!

Janet Morrison RN BScN, Advanced Diabetic Foot Care Nurse

Janet joins Soft Soles as an experienced Clinic Educator for BScN nurses training at St. Lawrence College. Her passion for foot care was developed after looking after both parents and recognizing a need for compassionate foot care nurses in the community. Janet took the initiative to expand her education and knowledge to provide evidence-based foot care practices to those in need.


Her focus and passion are on prevention and maintenance with a warm caring family-centered approach. Soft Soles is happy to have her join our team!

Shuang yan Zhou, RPN, Advanced Diabetic Foot Care Nurse

Shuang yan came to Canada when she was 29. She previously worked as a teacher in China, gaining experience building respectful and professional relationship with clients. Upon arriving in Canada, she first worked as import and export agent in Montreal and then ran her own restaurant in Ottawa.


These positions required her to effectively communicate with clients in English, French and Mandarin. Since 2017, she has been working as a registered practical nurse with specialty in geriatric care. While working in this position, she gained an appreciation for the importance of the foot care. Because of the professional interest, Shuang yan joined Soft Soles as an advanced diabetic foot care nurse to promote foot health.
